QUESTIONNAIRE: Please complete and return the enclosed Meeting Questionnaire in order to achieve a more personalized/pertinent presentation. AV REQUIREMENTS: If the presentation is to be from a stage or podium with space to move around, a clip on lavaliere mic or a hand held cordless microphone is preferred. If the presentation is from a luncheon or banquet setup, or if for any reason space is limited, a lectern-mounted microphone is preferred. In either case, a lectern must be provided. If possible, an American Flag behind the lectern is appreciated. INTRODUCTION: The enclosed introduction has already been customized -- in varying degrees -- to your specific industry. However, any tasteful personalization of an introduction is always nicer. Please consult with Captain Coffee on your ideas. RECORDING: Beyond Survival is a copyrighted presentation. AUDIO or VIDEO recordings must be pre-arranged with Captain Coffee. If the recording is for resale, a pre-determined fee will be charged. Captain Coffee reserves the right to approve the product, and a master copy of the recording should be sent to him, for his files, as soon as it is available. PRODUCTS: Products are never mentioned from the platform. Captain Coffee thinks that for what he does, it is neither dignified nor appropriate. He does find, however, that after he has spoken, many people in the audience may inquire about the availability of his book, audio or video cassettes so they may share Beyond Survival with their family, friends, or associates. Therefore, mail order forms are made available at the registration desk or at a table in the meeting room (or wherever you desire) for this purpose. It is helpful if the Emcee, or whoever closes that session, simply mentions their availability. In order to enhance the specific take-home value of Beyond Survival, the book and cassettes can be pre-purchased and mailed to your meeting site. ARRIVAL/DEPARTURE: Every effort will be made to furnish specific details no later than two weeks prior to your date.
